Wearing a Mask to Your Appointment During the COVID-19 Pandemic

Following the Centers for Disease Control’s guidelines and statewide mask mandatewe require that patients and support persons wear a mask to clinic appointments and leave it on for the duration of their visit.  This is for the safety of patients and staff alike. We do not have masks at the clinic at this time, so please try to remember to bring your mask to each visit. Please talk to your provider if you’re unable to wear a mask.

We are gratefully accepting donations of handmade cloth masks for patients who don’t have access to one. Masks can be dropped off at — or mailed to — our Maple Grove office:
